Initial Comments:
An off-site state licensure survey completed on September 21, 2021 found that Lehigh Valley Home Care Hazleton had corrected the deficiencies cited under 28 Pa. Code, Part IV, Health Facilities, Subpart G. Chapter 601. The deficiencies were cited as a result of a state re-licensure survey completed on July 23, 2021.
